In-Clinic Care Team

When you visit Southland Integrated Services, you are never navigating care alone. In addition to the support provided by your medical, dental, and behavioral health providers and clinical staff, other members of our care team are here to guide, support, and connect you with the services you need. It’s all part of the comprehensive care you receive at Southland.

Patient Support Specialists

Our Patient Support team is often the first friendly face or voice you meet at Southland. They are here to welcome you, help you check in, answer questions, and make your visit as smooth and comfortable as possible. From scheduling appointments to updating information Patient Support is here to make sure you feel cared for every step of the way. 

Patient Navigators

Our Patient Navigators are here to help you move forward with your care. They work closely with you and your care team to help coordinate services, connect you to resources, and support you in understanding next steps. Whether you need help with referrals, follow up care, or community resources, Patient Navigators are here to help remove barriers so you can focus on your health.

Beyond the Clinic

At Southland, care goes beyond the walls of our clinic. While this level of support is not always available in a traditional community clinic setting, Southland offers additional services that help patients navigate needs outside of their medical, dental, and behavioral health visits. By connecting patients with resources, support, and services beyond the clinic, we provide truly holistic care that supports the whole person.

Case Managers

Our Case Managers work closely with patients and families to provide personalized support for life’s challenges that affect health. They help connect you to resources such as housing support, chronic care management, benefits assistance, and community programs. Case Managers take the time to understand your situation, help you set goals, and navigate complex systems. Their focus is on stability, independence, and helping you move forward with confidence.

Program Support Staff

Our Program Support staff help keep services connected and accessible behind the scenes and in the community. They support special programs focused on chronic conditions, care coordination, education, and access to essential resources. By working closely with community partners, Program Support helps ensure that care continues beyond your clinic visit and into your daily life. 

Cost & Financial Supports

Medi-Cal

Medi-Cal provides free or low-cost health coverage for eligible California residents, helping individuals and families access medical, dental, behavioral health, and other covered healthcare services.

At Southland, we understand that applying for health coverage can feel complicated. Our team is here to support patients with the Medi-Cal application process, answer questions, and help connect you with available resources.

Medicare

Medicare is a federal health insurance program primarily for adults age 65 and older, as well as certain younger individuals who meet eligibility requirements. Medicare can help cover services such as medical visits, preventive care, hospital care, prescription medications, and other healthcare needs.

At Southland, our team is here to help patients understand Medicare coverage and provide support with the application and enrollment process. We can answer questions and help connect you with available resources.

Insurance Made Simple

We help you understand your health insurance, including what’s covered, what you may owe, and how to use your benefits. Our team is here to answer your questions and guide you every step of the way.

Sliding Fee Discount Program

WHO QUALIFIES

Every patient is invited to apply. Eligibility is based on family size and household income.

  • Open to all patients across all clinic sites.
  • Recertification is required every two years, or sooner if your income or family size changes.
  • If you cannot provide income documentation, you may complete a self-attestation form.

WHAT TO BRING TO APPLY

  • Photo ID for the head of household (any government-issued ID is acceptable — driver’s license, passport, green card, permanent resident card, etc.). If you do not have formal identification, give us a call for further support.
  • Proof of household income (2-3 most recent pay stubs, Social Security or unemployment letter, self-employment ledger, or a signed self-attestation form).
  • Proof of household size (tax return OR a signed declaration listing dependents)
  • If you have insurance, your insurance card. If you have no insurance, that is OK — we will help you apply for Medi-Cal, Healthy SF, or Covered California if you qualify.
  • English, Spanish, and Vietnamese enrollment counselors are available at every clinic to walk through the form with you.  We can assist with other languages as well with prior appointment, just give us a call.
  • The application takes about 15 minutes.

HOW TO APPLY

  • Walk in to any of our our clinic locations during business hours and ask the front desk for a sliding-fee application.
  • Or call (714) 620-7000 (Monday – Friday 8 am–5 pm) to be connected with an enrollment counselor.
  • Existing patients can also bring documentation to their next appointment, just inform our Patient Support Specialist.

GOOD FAITH ESTIMATE

If you do not have health insurance or choose not to use it, you have the right to receive a Good Faith Estimate of expected charges for scheduled or requested services. You can ask for an estimate when scheduling, at the front desk, or by calling (714) 620-7000. You can receive an estimate immediately if you are able to provide household income and number upon inquiry. 

Submitting Your Payment

We make it easy and convenient for patients to submit their payments. You can pay securely online through our patient portal, by phone with our billing team, or in person at any of our clinic locations. No matter which option you choose, our goal is to provide a simple payment process so you can stay focused on your health.
